Modifications need a referral from an Occupational Therapist. You can phone Southcare on 02 9540 7540 to arrange an Occupational Therapy assessment. Alternatively you can contact a Department of Veteran Affairs Occupational Therapist if you are a Veteran.

An Occupational Therapist is a health professional skilled in specifying requirements that are tailored to the needs of a client. Close consultation and cooperation is maintained between the client, home modification service and the referring occupational therapist.
